An evidence-based practice (EBP) paper walks a US nursing faculty reader from a clinical question (PICOT) through a systematic literature search, an evidence table classified by Melnyk levels of evidence, a synthesis of findings, and a proposed practice change with an implementation plan. Below is the full structure used in BSN, MSN, and DNP courses.
Section 1 — PICOT question (150-250 words)
Population: age, condition, setting. Intervention: the specific practice change. Comparison: current standard of care. Outcome: measurable clinical outcome. Timeframe: how long to evaluate. Example: "In adult ICU patients on mechanical ventilation (P), does chlorhexidine oral care every 4 hours (I) compared to standard oral care (C) reduce VAP incidence (O) within 30 days (T)?"
Section 2 — Search strategy (200-400 words)
Databases: CINAHL, PubMed/MEDLINE, Cochrane Library, and one specialty database (e.g., PsycINFO). Boolean strings: PICOT terms combined with AND/OR. Filters: peer-reviewed, English, last 5 years for interventions and last 10 years for foundational theory. Report exact search terms and hit counts — faculty grade the reproducibility.
Section 3 — Evidence table (Melnyk levels)
Level I: systematic review / meta-analysis of RCTs. Level II: single RCT. Level III: quasi-experimental. Level IV: cohort or case-control. Level V: systematic review of qualitative/descriptive. Level VI: single qualitative or descriptive study. Level VII: expert opinion. Aim for at least 6 studies with a mix of Levels I-III where possible.
Section 4 — Synthesis (400-800 words)
Do NOT summarize each article separately (that's a lit review, not a synthesis). Group findings by theme, note where studies agree and disagree, and identify the practice implication. End with the strength-of-evidence statement using the Johns Hopkins or GRADE framework your program specifies.
Section 5 — Implementation plan (400-600 words)
Use the Iowa Model, Stetler Model, or ARCC Model — pick one and cite it. Include: stakeholders, timeline (usually 3-6 months), pilot unit, education plan, outcome metrics, and how you'll evaluate. Faculty look for a real-world plan — not a wishlist.
Section 6 — APA 7 hygiene checklist
Running head only on title page (student paper). Level 1-3 headings correctly formatted. In-text citations with narrative and parenthetical variety. Reference list with DOIs on every journal article. No secondary citations for primary sources.
